Paraphimosis is a condition in which the foreskin, once retracted behind the glans, cannot be returned to its original position. It must not be confused with phimosis, in which the foreskin is so tight that it cannot be retracted. Your description is a little ambiguous, please clarify whether you can retract your foreskin.
The danger in paraphimosis lies in the foreskin forming an extremely tight ring of tissue that impedes blood flow along the penis, and may result in swelling and gangrene. Patients who suffer from this condition often have penises which are swollen at the glans but flaccid along the shaft. If your penis, on the whole, can stay erect normally, then you probably just have a tight foreskin and are not really suffering from paraphimosis.
However, if you constantly experience pain when you retract the foreskin at erect state, then you should seek professional advice as it may cause you discomfort and affect your performance during sexual intercourse. A tight foreskin may also increase the tendency for bacterial infection if you are unable to wash the area behind the glans for hygiene purposes.
