Question regarding Lost IC

  Anyone know if there is max. no. of times we could afford to lose our IC? Being very careless I've lost my IC for the 3rd time. May I know if what kind of penalty Of which i'd b given when i go report? Because I heard there is a maximum no. of time of which we are allowed to lose our IC
