Any Hard Rock/Heavy Metal bands who have never changed line up since their start till their end? I've been pondering on this, can't think of any.

Just checked it out, Queen. They have never changed members and even after Freddie's death, there wasn't anyone hired to replace the irreplaceable. And if you're thinking about Paul Rogers, he's just one of their Queen + ... Projects. John Deacon too, he wasn't replaced, they just had a session musician in a Queen + ... project.
So Queen, first on the list. -

Mantas, before Death was created. Also no changes in their line up until they changed name. They had an unknown bass player who only played in their first recording, they remained bassless until Death.
