from my observation, it appears the middle axle for most tri-axle buses is really free-turning.

heat from within the engine is removed by passing coolant water through pockets running between cylinders in the engine. the heat retained by the water going out of the engine is then "blown away" by the radiator.
the hissing sound is made by..... the compressed air tank. as the air compressor continually compresses air as the engine runs and feeds it into the air tank, the pressure inside will increase so the tank will routinely release some compressed air out together with moisture that will accumulate inside the tank due to condensation (the air compressor is hot with all the running).
