You missed out:
Services 40, 103, 104, 134, 183, 261 and 382 using NAC DD.
Service 84 using VO 2-axles and superbuses.
I don't remember I saw DD on services 63, 96, 131 and 155. -
Sv. 131 never used DDs due to low-lying trees along Henderson Rd. Same for Sv. 124. You missed out Sv. 385

Weren't the DDs for 96 and 151 trials only? 63 got DDs meh? -
Service 269 also used Leyland NAC DDs on its last days serving in Singapore.. It also used the Leyland Lynx (SBS3572Y) before for a short while, shuttling between 267 and 269..

Service 41 plied between Bedok and Bt Merah interchanges, it was later integrated with service 197 (former Jurong interchange - Lor 1 Geylang).
Service 4 in the beginning plied between St. Michael's and Bedok, later extended to Pasir Ris via former service 19 route. Later it was cut short to Tampines. It was finally integrated with service 21 (Pasir Ris - Lor 1 Geylang). Service 67 (Choa Chu Kang - Tampines via Jln Kolam Ayer and PIE) was amended to cover the part of service 4 route between Eunos MRT and Tampines interchange.

When did sv 261 and 382 uses double deck? My only recollection is that for AMK, is sv 269, for Eunos, it's sv 385.

I can SWEAR I've never, ever see a DD on Service 40. And with the condition of Siglap Rd, it is absolutely impossible for a DD to ply that road.

This is what I remembered, if my memory isn't failing.
However for Services 382 & 385, only the latter operated DDs. And the DDs used was the MBO305s before being replaced by the VO 2-axles. What I understand is that, HGDEP never operated the LO 2-axles, at the point of time Service 385 had DDs, that is.
