What is the format to write a statement?

My sergent wanted me to wite him a statment regarding a mistake i made.
If im not wrong,there is a standard format to writing a statment right?

something i wanna add, not sure if it;s correct.
the statement of the incident, just write pure facts, dont include yr interpretations of anything, include specific time and date of any event.
i see some statements then to include nonsense events and those relevant ones are left out or briefly write nia -
This whole statement thing i think some genius warrant thought of this fanciful idea for soldiers to document their mistake.
