Put off duties for a while and sent for counselling. Monitored 24hrs. No joke man, last time I was involved in my unit's mono intake, and got this joker who wrote a suicide note at home before leaving for intake. Parents noticed it about 2 days later, and made a big hoo-ha. Everyone had to give special attention to him.
Please don't pull this kind of stunt, cos alot of people will kenna additional things to do. -

Its chargeable offence both civil and military i think. correct me if im wrong. I read thur a case this serviceman kena charged for trying to commit suicicide n he kena charged. I dun know what will happen if u do declare. However, if u have recovered from ur past attempt and have a positive outlook on life and on ur NS. I dun think it will matter.
However, (no offence but I have to say it) if u declare under a pretext of attempting to get downgrade or getting special treatment then be warned: SAF is not a organisation u can play around wif. -

my father commited suicide leh . ( depression )

family history mah , maybe i have this problem too .
my ah gong died of stroke and my grandmother kidney failure .
my uncle have night blindness .
some have diabetes and some have hypetension .
some have hypotension ( low BP ) , some have cancer , some have kidney stones , some have contaract , my brother have phimosis ( thick fore skin ) , some is autistic ................................

if ur questionaire got state lots of medical probs in ur family prob the MO will ask more questions...but the interview wif MO last very short one...mine was like 5mins the max cos they rushing for lunch! the whole checkup last at least 4 hours if u go cmpb early...inclusive of the IQ test,but if go like 10am will take more time like 6hrs cos of long queue waiting to do the IQ tests..the medical components very fast over de
