surgery due to medical grounds and medical claims

  i was told abt making medical claims for my injury which is permanent and led to my downgrade.
  
  if i were to go for surgery and it's treated successfully, am i supposed to be upgraded or remain?
  
  on the same issue, my diagnosis was at least 1month plus, so how do i make a claim ? my spec says i must pay for the report, which isnt cheap.
