As stated, my medical checkup is next week, but i just realised i've lost it.
I hand it over to my mum for safe keeping together with our whole family birth certs and etc, but when i asked for it today, we cant find it anywhere.
I cant seem to print it again from the website. Do you guys know what am i supposed to do?
You are not the 1st nor the last who will loss that medical questionaire or forget to print it.
You should be able to get another copy at CMPB when you go for your medical checkup but i'm unsure what they will do about the part the requires parental signature.
Another alternative is try call up CMPB and ask whether there is anyway they can sent you the form or let you download it again so you can bring it with you next week.
What i'm thinking is if i still can't find it by tonight, I'll type out a declaration letter stating that my guardian acknowledged the lost of my questionnaire and sign at the bottom.
One to state that my mum know i've lost it.
Two to confirmed that the form i've submitted to them is of the truth.

That might work but it might be easier to just call them up and ask whether its possible for them to get you another copy of the form so it would be easier for you as well as them since you still need a hardcopy of the questionare when you meet the MO at station 6.

Hi isoboney,
Have one of your parents go down to CMPB with you. It is likely that they have spare copies of the questionnaire, in which case you can have your parent sign on the spot for you.
On the other hand, have you tried searching through the personal documents one piece by one piece? Perhaps the questionnaire is stuck between two pieces of documents or within a file/folder at home.

Hey thanks so much for those who gave me advice on this matter.
CMPB sent me a link whereby i can download the form, print it and fill it in myself.
But that can be used ONLY after you have registered for NS.
