I've recently had a some medical problems regarding my skin after participating some activities. I went to the MO which got me referred to a specialist. Now, i am referred to the SAF specialist.
I just recieved this "Injury Declaration Form" to fill in. It says that i DO NOT WISH to pursue any compensation claim for my injury, including free medical benefits and compensation from the government if i was to sign this form. And, somehow, i was told to fill up and sign the form.
So, what exactly is this declaration form about? Should i sign it?
I am supposed to hand in the form, SIGNED, by tomorrow. SOS~
It is an indemnity form, and it is totally different from an in-service training injury report.
If you sign it, you cannot claim any compensation money from Mindef anymore.
If you don't sign it, you still can claim for it should you pursue the matter further.
