Just went for my mdeical checkup but realised i forgot to give the pink form before leaving. It's the one with all the signatures of each station and says "NATIONAL SERVICE PRE-ENLISTMENT DOCUMENTATION & MEDICAL SCREENING CHECKLIST".Important or not?
Govt matter are best not to mess around with.
Better you take them back to play safe.
