It CAN work that way. Just because it hasn't been done does not mean it cannot be done in future.
The very fact of knowing that the full medical costs would be shouldered by the individual can be a huge deterrance to those who are considering picking up the habits. -

i have an insurance policy which i pay for faithfully.. so 20 years down the line, i get lung cancer.. they dig up the past, saying i used to smoke 20 years ago, and refuse to pay up..
in the end, you will have these self-righteous people who condemn everyone... i have a heart attack because of my preference for red meat.. and i don't get subsidies?

It may be too late to start such a policy now, and even unfair to revise current policies, but, a new policy can be implemented in future, to deter young people from picking up the bad habits.
It is NOT a matter of self-righteousness. Rather, it is a matter of if you fully understand the risks of indulging in a bad habit and you STILL insist on going ahead with it and take the risks involved, then fine, bear the full risks!!
Smokers should not be covered for any respiratory illnesses. And drinkers, all hepatic-related illnesses. -

You can still restrict the policies to affect only smokers and drinkers. Because those two are clearly habits that remain undesirable. -
