Its a very strange case.
Its quite hard to imagine how someone can get caught between the winch and rope because the procedures are in place to prevent this from happening. And its one of the first thing a seaman is taught to do and safety emphasis is extremely heavy due to dangers inherent.
Exactly what do they mean by "equipment malfunction"? Only thing I can think of is that the winch moved when the poor sod was wrapping the rope around it.

Its really the last thing you can imagine, happening to someone you spend long stretches of time with.
The RSN is a small place and my fb feed is now flooded by them requesting for blood, which obviously the guy lost a hell of.
