Mispronunciations that really irk me are 'salmon' and 'sword'.... a lot of people don't seem to know that the 'l' is silent in the former, and the 'w' is silent in the latter.

Differentiating between the two is made more difficult when the consonant in can't isn't adequately emphasized or difficult to hear (say, in a noisy environment).
Nowadays, if I am not sure if the speaker said can or can't, I'll follow up with, "Did you mean can or cannot?"
