  ok this is abit complicated and isnt just about the literal meaning when you are talking about medicals..
  
  wai ke would mean those departments that involve surgury.
  
  Nei ke is anything that doesnt.
  
  Eg:
  shen2 jing1 wai4 ke1 means neurosurgury, shen2 jing1 nei4 ke1 would most mean psychiatrist.
