  Dear all,
  
  I would appreciate it if you would help me complete this survey regarding the issue on "Food Service Trays"; the ones you see in fast food restaurants and food courts!

  i got comments on ur survey questions
  
  1. always leave the age n gender part as the last 2 qn.. sensitive questions like these should be asked last
  
  2. for ratings kinda questions make sure the choices are even numbers.. so there's no middle number for u to choose like 6 in ur current questionaire.
  
  if everyone choose 6, den it's a tie liddat lor no good.
  
  if u wan them to rate 1-11 on the list say so.. -.-
  
  for 13, i think make them rate how important each component is will be better...
  
  i can't put 1 for all of them -.-
  
  make it a different colour will be better... like red for those u've selected already
