The following stress checklist has been done by me and my team for a project last semester. This is compiled and summarized from a survey of 150+ people, of which 70+ came from sgforums.
This checklist includes the affective, physical and behavourial symptoms of stress. It is designed as a quick indicator if your stress level is high. It helps you to recognize some symptoms that determine your stress level.
To use the checklist, assign a number from 0 to 5 to each statement below, 0 being never happened, 5 being having happened severely.

0-30 Good!
Congratulations, your stress level is very low! It is at a healthy level.
31-60 Average
It would be good to lower your stress level.
61-90 Bad
Modify your lifestyle to reduce your stress.
91-120 Dangerous
Immediate action is required to reduce stress!
