Muscles don't convert to fats as a bodybuilder stops working out. What happens is that the muscles will undergo atrophy (grow smaller) and so there will be extra skin around it, which will sag.
Anyway, if you look at the photo where he stands with the girl, the shadow seems to fall at different angles. The shadow of the lady's head falls at a steeper angle than "Arnold's" head shadow. I'd say its probably a fake.
Besides, thats not his wife.... -
