  Why not just find one of those poll/votes websites, put in your questions and then give us the link....

  It's your choice. Maybe you prefer to tabulate the results on your own instead of the website doing it for you.
  
  In the event you're giving a presentation based on your survey, having the results on a web-site gives you a more professional look rather than something you wrote on your piece of paper.
  
  No pop-up ads. I did a project before and needed a survey and asked people here to help me did it. You can dig up my ol' thread if you want.
