Well,she's considered to be the old buddha to be worshipped like god like that by people throughout china and no one can stop what they want to do. Absolute power they held. they had everything wanted thus life somehow got bored for them.
So they like to torture people for entertainment,i ve even heard stories like gorging out the eyes and peeling out flesh piece by piece until the fellow dies

In order to climb to power certain sacrifices is needed. Being ruthless is one of the charecteristic needed to become a leader in trouble times, just look at Hitler, Starlin or even Churchill. Though i pity those who had suufered under her.
