hi guys? may i know where to download medisave authorisation form? cos i screwed it up and i tried to search for a new piece to download online but i cannot find the normal form. i found 1 form but have the SGH logo on the top right corner. i cannot download that one cos i need a Changi general hospital de. and the top right hand corner is not any hospital logo but is account no. and medisave bal. can anyone tell me where to download the medisave authorisation form? THANKS!!
go to the hospital registration/admission counter to get a new one and fill in on the spot lah.
