read more, as already mentioned by many others in the forum.
and go through what you just wrote and find out the possible errors, same goes for speaking, go through it once in your mind before speaking and make a conscious attempt to remove the errors before u speak. -

Personally suggest read more newspaper and cover it with your dictionary.. See which parts of the English paper you're weaker and WORK on them...
MAKE SURE you have ADEQUATE REST and SLEEP before your PAPER...

-grammar,comprehension: Read more Newspaper,magazines.. Don't read too much blogs..
-listening: watch more Channel news asia,listen news radio more.. like 93.8 fm...
-Speaking: Speak english more and watch your tongue... try to avoid singlish... Doesnt mean that u should talk with US or british accent like "Good day mate..." but just simple "how are you?" instead of "you ok ah"
Listening n singing english songs in ur bathroom may help also..
Definitely this is boring... keep watching news, listening news radio... but think about it.. if u wanna improve, u have to be consistent... -
If you must read the Straits Times, avoid the HOME section if you can. It feels and reads like a training ground for cub reporters. Not sure if there is an editor on duty here.

anyway...I recommend reading more.I find it more useful than British Council...if U dun want to read...I personally think its difficult to improve...but U can try listening to english music stations like what people here posted...may help...I dunno...neber tried it...
