  You need to put your real name, NRIC number, phone number and home address.
  
  They will only put your name in, no pseudonym unless it's super sensitive issues (very very rare).
  
  Sometimes they might call you to verify some points - which is why the phone number is necessary.
  
  How I know? I have done this many times in ST, Today, Streats, NP - but not recently though.... boh mee eng.
  
  Don't be disappointed that your letter may not be posted. My hit rate is less than one quarter.
