Wanna apply for a job. Then realised that I misplaced my O lvl cert. F***
Any idea where 2 go replace and need to bring what along? -

Purchase of Statement of Results
Ever found yourself looking for your educational certificates, only to realise that you may have thrown them out during your last spring cleaning? Well, don´t despair. If you have lost or misplaced your PSLE, GCE ´N´, GCE ´O´ or GCE ´A´ level certificate(s), simply drop by at our Customer Service Centre, call our Call Centre or e-mail us to request to purchase a copy of your statement of results.
You will be required to come down to our Service Centre to make the purchase. Please bring along your NRIC as well as S$10 for each statement that you require. If you are unable to collect the statement of results yourself and would like to appoint someone to collect it on your behalf, please furnish your proxy with a Letter of Authorisation. Your proxy should also bring along your original NRIC as well as his/her original NRIC when purchasing the statement of results.
You can also request to purchase the statement of results by mailing in the application form. The application form should be accompanied by a crossed cheque payable to 'Accountant General, Singapore' and a photocopy of both sides of your NRIC. Please note that an additional charge of S$2 (per local address) and S$5 (per overseas address) will be levied if you wish to have the statement mailed to you or the intended recipient. You can download the application forms to purchase the statement of results here. Please mail the form to: Customer Service Centre, Ministry of Education, 1 North Buona Vista Drive, Singapore 138675. -
