or even maybe the phone dropped out of your bag? zip not zipped up, vicro not attached, or in your haste you thought you push you phone into your bag's side pocket but it didnt slid in but fell out instead?
there are many possibilities. not saying you are in the wrong, but you might have been careless. learn to either leave your phone in an inner zipped compartment, or leave it in your pocket where you can feel it all the time.
