for the "submit" page, u need some programming in php, asp, asp.net, jsp, vb, etc...
Mi advice is...go find those free web survey site and then create there...simple and easy... no point doing the programming, unless the programming is part of your project lah -

pple use MySQL or Oracle is when they hav very large sample size, plus the hit rate is damn high, meaning at one time (in a split second), at least 50 pple are accessing the database.
For your case, u need a static webpage for the survey page, and then a database add page to add the submitted data into the database.
Access allows a 5-concurrent hit access ... so should be no problem lah...
well, if u hav MySQL, so be it lah...

Line 1: tell the computer where is the MySQL server is located (u got to change the phpie.com to the one u have...it could be an ip address.
Line 2: MySQL_User is the userID of the MySQL server that u use
Line 3: if you dunno this, I kock your head ah
Line 4: Is the name of the database that u have created
Line 5: is the table name that you created
Line 6: the connection statement to use...just use the same as shown, no need to modify
Line 7: this is to effect the connection
Line 8: The SQL statement to insert the data into the relevant table
Line 9: execute the query statement (to insert the data)...and returns a "true" value if the data are properly inserted, else it will be "false" in the variable result

e.g. Name in the Mysql database
Then make a simple form: one textbox to type in the name, and submit to a data_add.php page where u try to input this name into the database...
then use another page to read the database to see if the name u typed is inside the database or not lah
"Select Name from Table_Name"
That will be another $2000
